DECLARE AND CONSET FORMALY

1) that I hold a valid medical certificate for fitness to practice scuba diving;

2) that I am in suitable physical and mental condition for diving, that I can swim independently, and that I feel comfortable in deep water; that I have never had respiratory, infectious, cardiac, neurological diseases, or surgeries that would constitute absolute contraindications for underwater activities; that I do not suffer from behavioral disorders, anxiety, panic attacks, claustrophobia, epilepsy, agoraphobia, loss of consciousness, or nervous system disorders; and that I will inform the Dive Center staff of any situation I believe may be useful to ensure my safety and well-being;

3) that I have not taken and am not taking any narcotic and/or psychotropic substances, that I am not under the effect of any medication, and that I have not exceeded the consumption of alcoholic beverages and food;

4) that I am aware of the risks, both predictable and unpredictable, associated with underwater activities and that I knowingly accept them;

5) that I am familiar with and will adhere to all safety regulations, provisions, and limitations related to the technique of sport and/or technical scuba diving, both before, during, and after the dive, taking into account the diving certification I hold, as well as my level of training and experience;

6) that I will participate in the pre-dive briefing, ensuring I understand all the information related to safety, the path to follow, the correct behaviors to maintain throughout all phases of the dive, the signals used, communication with the guide regarding my air consumption, the ascent procedures, and safety procedures. In case of any doubt, it will be my responsibility to ask the guide for clarification;

7) that I will observe and respect the rules of the dive center, national laws, and the instructions given by the boat staff and the safety coordinator regarding descent and ascent operations;

8) that I will conduct all dives within the limits imposed by the safety curve, unless I hold a certification that allows for decompression in planned dives with such a procedure;

9) that I will conduct all dives with suitable equipment and gear that is in good working condition and properly maintainence;

10) I am aware that diving activities may also take place in locations far from centers equipped with hyperbaric chambers and competent medical assistance, and I accept to participate at my own full responsibility. I authorize the dive center staff, if necessary, to administer therapeutic oxygen to me;

11) that I will not engage in any type of fishing during the dive and will not collect objects or marine organisms;

12) that I take full responsibility for any personal injuries and/or damages caused to others due to my behavior that is not in accordance with the good diving practices or is objectively irresponsible;

13) that I am aware of and accept that the scuba diving guide service is limited to simply accompanying me to the dive site and excludes any scuba training activities and/or other activities related to scuba instruction;

14) that I am aware that failure to comply with the provisions, regulations, and directives of the dive center staff will result in the termination of this contract, with exclusion from all activities, whether ongoing or future;

15) that I have carefully read and accepted the dive center’s regulations, as well as the content of this document, and that I have clearly understood the meaning of each point before signing it.

For the purposes of Articles 1341 and 1342 of the Civil Code, I understand, agree to, and expressly accept all the points and objectives of these rules established for the safety of myself and all participants in the dive.

Privacy Policy

The undersigned authorizes Argentario Scuba Point to process personal data, including those considered as special categories, for the purposes of membership/registration/reservation for the services of the Dive Center, as well as for administrative and accounting purposes. To this end, the undersigned declares to have read the Dive Center’s regulations and the specific regulations, and thus acknowledges having been informed about the data processing methods and their rights to request information about the processing, including the right to rectify, update, and delete their data. We inform you that your choice is voluntary, but without consent to the processing, we will not be able to provide our servicesi.
