The undersigned hereby declares that he/she is in good general health and in particular in a condition suitable for scuba diving, that he/she is aware of the risks involved in this activity, that he/she dives within his/her licence and experience limits and that he/she respects the instructions and directives of the diving guide or instructor who will accompany him/her on the dive.

In accordance with EU Regulation No. 2016/679, we would like to inform you that the processing of personal data provided in relation to participation in diving activities is exclusively aimed at managing these activities and is managed by Blu Infinito.
The data collected through even automated procedures will be used in the ways and within the limits necessary for the purposes described above and will be made available to the owner of Blu Infinito and the data processing manager.
Providing data is mandatory and necessary for enrollment in courses and dives. Failure to provide the requested data, especially sensitive data regarding health status, will result in the inability to organize the activity.
In accordance with the EU Regulation, you have the rights of access, rectification, updating, erasure of personal data, as well as the right to object to their processing for legitimate reasons. You can exercise these rights by contacting the data processing manager at the email address provided above.

Release and Right to Image
I authorize the filming of my image in photos and/or videos during diving activities organized by Blu Infinito, consenting to their dissemination on Facebook, Twitter, Instagram, and other social networks connected to Blu Infinito, as well as on the website.
I declare that I release Blu Infinito from any liability regarding the improper use of personal data and photos by third parties.
In accordance with GDPR 2016/679, the undersigned may revoke the authorization for images at any time by contacting the Data Controller.
